Codornices Veterinary Clinic
Before your visit:
We will be allowing one client per visit (multiple pets still equals one person) inside of the building for exam appointments only. Technician appointments and drop-offs will still remain curbside. We still ask the following of you:
- Plan to arrive 10 minutes early.
- Call us from the parking lot, telling us where you are parked (please park as close as possible to our front door), the basic make/model/color of your car, and the number you are calling from (you must bring a cell phone).
- Wear a face mask (N-95 or KN-95).
- Bring your proof of vaccination to show our staff for entry.
- Two to three days prior to your appointment (or right now if you’ve made a same-day urgent exam or a last-minute general appointment for today/tomorrow), our veterinarians need you to fill out the below Exam Questionnaire Forms (and yes, we still need these completed, even “just” for recheck exams).
- Note: the veterinarian requires that these forms be completed and submitted before we close the day before your scheduled appointment. (This is for exams; if you have a technician appointment, you do NOT need to fill it out.)
- Our technicians will then come to your car and get one of you and your pet.
- The beginning of the exam will take place with you in the room. Once a treatment plan is decided, you will return to your car while we carry out the plan (in other words, your pet will still be with us). (Please note that if you still prefer socially-distanced exams, just inform the Receptionist upon check-in.)
- For those who opt for socially-distanced exams: Make sure that you have a pen and pad at the ready to take notes when the veterinarian calls. Our staff requests that you PLEASE have the ability to take notes when the veterinarian calls you on your cell phone during your pet’s appointment.
- The veterinarian will then call you and discuss any findings or treatments. (Answer any call that you may get from “unknown” or “blocked” numbers during your exam, as it may be us.)
- After the exam and any needed treatments are complete, the technician will return your pet to your car.
- At that point, while still in our lot, please call us from your cell phone, and we will process the payment via credit card over the phone, or wait for us to call you – but please do not leave until your invoice is paid.
- We’re sorry, but at this time, we’d like to avoid taking any checks or cash whenever possible.

NOTE REGARDING SOCIAL DISTANCING:
Please note that during this time of Social Distancing, we will be doing things a bit differently to limit human-to-human contact, for everyone involved.
- Plan to arrive 10 minutes early
- Call us from the parking lot, telling us where you are parked, and the basic make/model/color of your car, and the number you are calling from. (You must bring a cell phone.)
- Wear a Face Mask. (New requirement by the County of Alameda)
- Please fill out the included visit questionnaires (above) and email them back to us at [email protected] before your scheduled appointment time. (We are including both a dog and cat one – just fill out the one that pertains to you.)
- If you cannot fill out and email the form back, our receptionists will ask you all of the questions over the phone. (Due to the time this takes, please help us avoid this step by submitting the below forms.)
- Our technicians will then come to your car and get your pet.
- The exam will take place in the hospital while you wait in your car.
- The veterinarian will then call you and discuss any findings or treatments. (Answer any call that you may get from “unknown” or “blocked” numbers during your exam, as it may be us.)
- After the exam and any needed treatments are complete, the technician will return your pet to your car.
- At that point, while still in our lot, please call us from your cell phone and we will process the payment via credit card over the phone.
- At this time, we’d want to avoid taking any checks or cash whenever possible.
New Clients/First Exams
PLEASE NOTE THAT PATIENTS COMING TO US FOR THE FIRST TIME SHOULD COME IN 20 MINUTES EARLY (having already completed and returned this form), IDEALLY WITH THEIR PREVIOUS MEDICAL RECORDS ALREADY EMAILED TO US.
